The Altar Emerald was a location on Angel Island and is the location of the Master Emerald.

For an unknown length of time, the Master Emerald sat on the Altar of Emerald, where it was guarded by Knuckles the Echidna. Knuckles, however, left ythe master emerald alone and left the island to command the resistance during the Eggman War. Despite leaving the emerald alone, the emerald was presumed to have been hidden to prevent anyone accessing it. (StH: #2, #3, #8)

Battle for Angel Island[]
When Neo Metal Sonic (disguised as Eggman and acted as the leader of the Eggman Empire until the real doctor was found) discovered that Knuckles wasn't guarding the Master Emerald, he found it interesting.
After Sonic found out that Neo was acting as the leader of the co-ordinated badniks, Neo launched an assault on Angel island with the Egg Fleet, intent on capturing the using the now idle Master Emerald.
By the time, Knuckles, Sonic and various resistance traveled to Angel Island, they found Angel Island and the Alter Emerald enclosed in a structure built by Neo Sonic's forces, and heavily guarded. Neo Metal Sonic remade Alter Emerald into an enclosed chamber with the Master emerald being integrated into a throne for the robot.
